Preparing for Your First Visit
Scheduling an Appointment
When calling the office to schedule your initial evaluation, you will be asked your demographic, doctor, and insurance information. Be sure to have your doctor’s prescription, your insurance card(s) and any other pertinent information on hand to allow our office to best advise you of your outpatient occupational therapy benefit. Insurance coverage is constantly changing. To help understand your benefit, call your insurance company to find out what your outpatient occupational therapy benefit would be. | |||

Your Initial Visit
Please arrive 10 minutes prior to your scheduled appointment time to allow for completion of the proper forms: |
You Will Need To Bring On Your First Visit:
- Insurance Card(s)
- Doctor’s Prescription
- Insurance Authorization (if required)
- Referral (if required)
- Splints you are currently utilizing
- Any diagnostic testing results you wish to share with your therapist (x-rays, MRI results, EMG results, operative report)
What To Expect
During your initial visit to therapy, a full evaluation will be performed, and a customized program will be devised by your Occupational Therapist/Certified Hand Therapist. Special efforts will be made to answer any questions you may have. The patient's understanding and involvement in his or her rehabilitation program is of utmost importance, and participation in one's own care is vital. |
Exercises will be reviewed, both for your program in the clinic, and at home, and will be upgraded as you make gains in therapy.
Re-evaluations will be conducted at frequent intervals, and discussed with you. In addition, your physician will be notified regularly of your achievements in therapy, and in a cooperative effort, your program will be adjusted to optimize your progress.
